If you are a Mississippi Licensed Resident Agent and wish to write business through MWUA please complete this form (W-9) along with an Authorized User Form and submit to [email protected] . Please include a copy of your agency entity level, or individual agent producer level MS Department of Insurance license with the submission.
Please include all of your agency location contact information - primary phone contact #, primary office email contact #, fax #, mailing address for policy output/general correspondence, and mailing address for commission checks (if not the same address).
Note: The W-9 information is used for processing of 1099 tax reporting. Codes can be established at the individual producer level or the entity level. If at individual level, the W-9 should provide the individuals SSN, if at entity level the W-9 should provide the FEIN #.
Once approved, you will receive an assigned producer code with user names and passwords for authorized users via email.
